CORPORATE CARE
(BASIC SERVICES OFFERED)

1. Free short term counseling and referral (if indicated) for all employees and their immediate family members.
There is no limit on the number of counseling sessions available to employees or their family members.

2. Every employee has access to the home phone number of the President of Corporate Care for twenty four hour emergency coverage for all employees and family members.

3. All services totally confidential and professional.

4. Free Wellness and Education Trainings on topics of your choice. Trainings performed at the workplace as well as throughout the year at convenient locations.

5. Six convenient locations to serve employees and their family members. Morning, afternoon and evening counseling hours are available.

6. Formal referral process for under productive employees (with accompanying supervisory training).

7. Consultation services to assist in the resolution of problems
between co-workers.

CORPORATE CARE’S EMPLOYEE ASSISTANCE PROGRAM PACKAGE INCLUDES,
BUT IS NOT LIMITED TO, THE FOLLOWING:

1. EVALUATION OF PROBLEM, SHORT-TERM COUNSELING AND REFERRAL (IF INDICATED)
Evaluations of presenting issues are performed by qualified, master’s level counselors. When short-term counseling and/or conflict resolution is indicated, the counselor will work towards that goal. There is no set limit on the number of counseling sessions available. If a referral is indicated, it will be based on the individual’s needs and their approval. Approximately 15% to 20% of all individuals seen are referred for outside treatment.

2. IMMEDIATE FAMILY MEMBERS INCLUDED
Immediate family members include the employee, spouse and dependent children. Corporate Care understands that the present day family may not be reflected in the above description. It is our policy to be very flexible in determining “Immediate Family”.

3. 24 HOUR EMERGENCY COVERAGE
If an individual needs to speak with a counselor during normal working hours, they just call the EAP phone number and tell the secretary that it is very important that they speak with someone soon. The individual would be serviced ASAP, usually within an hour. If this happens after normal working hours or on weekends, they call the same number and and they will hear a message that will give them a choice of two numbers to call. The first number is for Clifton Springs Hospital Psychiatric Emergency Services. The other number is the home number of the President of Corporate Care. Individuals in crisis should never hesitate to call either of these numbers. That is made clear during the orientation sessions.

4. EMPLOYEE ORIENTATION SESSIONS
Corporate Care will present short information sessions to all employees. These sessions can be presented as many times as is necessary to reach all employees. This is very effective in letting all employees know of the benefits of this service.

5. SUPERVISORY TRAINING
We will provide trainings for all supervisors and managers to instruct them as to how to use the EAP to help deal with under productive employees. This training lasts approximately one hour. Topics covered include how to: recognize a job performance problem, document the problem behaviors, constructively confront the employee and access the EAP if necessary. There is also a video presentation to help managers grasp the procedure. These trainings will be presented as often as requested at workplace locations.

6. CONFIDENTIALITY
Corporate Care insures employee and family member confi-dentiality. All who seek counseling are guaranteed complete anonymity. Only by signing a formal consent to release information will any information be provided to anyone. Only specific and agreed upon information will be released. The client/professional relationship is protected by law.

7. REPORTS ON SERVICES DELIVERED
Corporate Care will provide reports concerning the following on a semi-annual basis (unless otherwise instructed):
A. Number of employees and family members using the service
B. Total number of counseling sessions throughout the period
C. Types of problems seen
D. Number of referrals made

8. OFFICE LOCATIONS AND OFFICE HOURS

Presently Corporate Care has 7 locations throughout the Finger Lakes area. These locations are: Geneva, Clifton Springs, Canandaigua, Newark, Penn Yan, and Auburn. Counseling sessions are available mornings, afternoons and evenings. These appointments may be scheduled by calling Monday through Friday between 8:30AM and 4:30PM.

9. PROMOTION OF SERVICES PROVIDED
Corporate Care will work towards employee knowledge of available services via: business sized information cards, brochures, posters for all locations, employee orientations, employee wellness trainings, and “Taking Care of Business” newsletter. Other methods will be utilized at request of employer.

10. WELLNESS TRAININGS
We will provide companies with free wellness trainings on topics of their choice, such as: Stress Management, Nutrition,Woman’s Health Issues, Drug/Alcohol Awareness, Parenting, Aids Awareness, or any other topic of interest. These trainings may also be offered at off site locations periodically throughout the year at convenient times. Additional trainings would be available for a negotiated fee. A partial list of wellness trainings offered is included with this proposal.

11. “TAKING CARE OF BUSINESS” NEWSLETTER
Three times per year we will publish a newsletter for all of your employees. These newsletters will remind your employees of the services available through the EAP and how to access them. Also included will be articles of interest on topics such as Parenting, Depression, Step-Families, Stress, Care of the Back and a host of other wellness related topics.

12. HARASSMENT TRAINING
Corporate Care provides, at no extra cost, a training on all types of workplace harassment. This includes sexual harassment, intimidation, verbal harassment, and other forms. This will be provided as frequently as needed or requested.

13. ON-SITE MANAGEMENT CONSULTATION SERVICES
An EAP professional will meet with any manager, upon request, to deal with individual or group issues at the workplace. We will work towards collaboration between management and the employee to establish a plan for problem resolution.

14. CRITICAL INCIDENT DEBRIEFING
In the event of a critical incident, Corporate Care will provide debriefing sessions for those parties involved. All human resources personnel will have the home phone number of the president of Corporate Care. If there is an emergency need after hours, just call him at home and he will see to it that someone will be there to help the employees cope with whatever has happened.

15. VIDEO BASED TRAINING ON WORKPLACE VIOLENCE
“Understanding and Preventing Workplace Violence” is a training designed for all managers and supervisors which includes topics such as: overview of workplace violence, predicting violent behavior, the firing process,legal considerations, warning signs and many others. This training is facilitated by Peter Pontius of Corporate Care.

IF A COMPANY CHOOSES TO INCLUDE THE FORMAL REFERRAL PROCESS, THE FOLLOWING SERVICES ARE INCLUDED
(FREE OF CHARGE).

1. SUPERVISORY TRAINING
All supervisory personnel will be offered training on the effective use of the EAP when dealing with an under productive employee. This training includes the following topics:
A. How to recognize job performance problems
B. How to document unacceptable job performance
C. How to “Constructively Confront” the employee
D. How to make a “Formal Referral” to the EAP based on job performance.

2. ON-SITE CONSULTATION FOR MANAGEMENT PERSONNEL
An EAP professional will meet with any manager, upon request, to deal with individual or group issues at the workplace. We will work towards collaboration between management and the employee to establish a plan for problem resolution.

3. FORMAL REFERRALS - INFORMATION WHICH IS SHARED WITH THE EMPLOYER
Corporate Care will provide the company EAP representative with the following information (provided a release of information has been signed by the employee):
A. That the evaluation time has been scheduled
B. That the employee did or did not keep the appointment
C. Whether a referral for additional services is necessary
D. Whether or not the employee continues with recommended treatment
E. That the employee has completed or failed to complete recommended treatment
